Short-term housing and support for Medi-Cal members recovering after hospitalization.
WHAT IS PANDO?
Pando is a massive grove of aspen trees, all connected by one root system underground. Though each tree appears separate, they’re one living organism. It’s a reminder that none of us stand alone. We chose the name to reflect that healing happens through connection, community, and care.

When MediCal members through Anthem and Mountain Valley Health Plan are ready to leave the hospital but doesn’t have a safe or appropriate place to continue healing, The Pando Home offers a short-term, supportive space designed to bridge the gap between discharge and long-term stability.

-Safe housing after discharge
-Supportive, healing environment
-Help navigating next steps
-Establishing roots in human connections
WHO IS ELIGIBLE
To be eligible for services, individuals must meet all of the following criteria:
Be enrolled in Medi-Cal through Anthem or Mountain Valley Health Plan (MVHP)
Be discharging from an inpatient facility such as:

Acute care hospital
-
Skilled Nursing Facility (SNF)
-
Psychiatric hospital or residential treatment center
-
Other licensed medical or behavioral health facility
-
Lack a safe or appropriate place to recover, including:

Experiencing homelessness
-
At risk of homelessness
-
Living in an environment not suitable for medical recovery

Be medically stable and not require 24/7 medical or nursing care
Be able to safely reside in a communal living environment
Be referred through an approved channel, such as:

Hospital care team or discharge planner
-
Managed Care Plan (MCP)
-
Enhanced Care Management (ECM) team
-
Primary care provider or case manager
-
Have a clearly documented estimated length of stay and an initial housing plan or recommendation

The Center for Violence-Free Relationships is a nonprofit organization serving El Dorado County and beyond with nearly 40 years of experience supporting survivors of domestic violence and sexual assault. Through trauma-informed counseling, legal advocacy, prevention education, and community outreach, The Center works to break the cycle of abuse and help individuals and families build safe, healthy, and empowered lives.
